What is Rhinoplasty ?
Complementing our expression, the characteristic organ of our face, the nose, is perhaps among our organs that are most exposed to aesthetic interventions today. Rhinoplasty operations, which push the limits of the title of “most applied plastic surgery” in the world every year, are surgeries that must be planned with precision.
Rhinoplasty requires very detailed and sensitive interventions. Since it is right in the middle of our face and we do not have the opportunity to cover possible errors, it is important that the surgeon who will perform the rhinoplasty is competent in their field and has encountered many different cases.
A little touch to our nose brings many changes. For this reason, each touch should be planned individually and the person’s own photographs should be used when planning the operation. At this point, we regret to state that your favorite celebrity’s nose is unfortunately not the right choice for every facial anatomy.
Rhinoplasty Preparation Process
In rhinoplasty performed under general anesthesia, not only the bridge of the nose is corrected, but also the intranasal cartilage structure is intervened. The nose wings are harmonized with the face, the tip of the nose is lifted. While these are being done, all the features of the breathing function, which is the main function of the nose, are preserved. While rhinoplasty is such a layered operation, the preparation process requires as much detail and precision.
First of all, as in every plastic surgery operation, the medical history must be shared with us during the consultation. Then, if you are using prescribed drugs regularly, you should share them with us during this process.
If you have blood thinners such as Ecopirin, Aspirin, Oksapar that you use in pill or injection form, you should stop using these drugs about 10 days before the operation, with the knowledge of your doctor who gave them to you.
Again, during this period, the use of all herbal teas and additional vitamin supplements, which are thought to have blood thinning effects, should be suspended.
Undoubtedly, smoking is another habit that should be interrupted during the preparation process. Smoking, which affects the breathing process seriously, should be stopped about 10 days before the surgery.
Our patients who want to have rhinoplasty are first given an appointment date for consultation. In this examination, the demands, changes and complaints of our patients are listed. An approximate view of the new nose form they will achieve is shown to the patients by making plans on the patient photos. In addition, patients are informed about which technique will be used in line with the needs of the patient and how the operation will proceed.
Today, there are basically two different rhinoplasty techniques. These are open and closed techniques. All the remaining topics such as let don rhinoplasty, piezzo rhinoplasty are the techniques under these two main headings. Both methods provide many advantages to the patient and the surgeon.
What Interventions Can Be Done With Rhinoplasty Surgery?
With rhinoplasty, the nose is made proportional to the face. The nose can be enlarged, reshaped or reduced. Nose wings can be reshaped. The nose belt can be corrected. However, the structure of the nose determines how much these procedures can be done. Factors such as thick skin and cartilage structure are the factors that affect the technique and intervention area of rhinoplasty.
Should The Closed Technique Or The Open Technique Be Used In Rhinoplasty?
Basically, one of these two techniques is used in rhinoplasty operations. Both techniques have their own advantages. Although the operations performed with the closed technique are reflected as “scarless rhinoplasty” today, there is no visible scar in the nasal aesthetic operations performed correctly with the open technique. However, the recovery process in rhinoplasty surgeries performed with the closed technique is faster than the open technique.
However, nasal aesthetic surgeries performed with the open technique have great advantages compared to the closed technique. In the operations performed in this technique, thin incisions are made in the lower part of the nose and at the intersection of the nose wings with the face. The nose skin is lifted backwards like a veil. In this way, the internal structures of the nose are seen much better than the closed technique. Since the cartilage structure and nasal internal structures are seen more clearly, there is a much more sensitive chance of intervention. In the open technique, after the necessary shaping is done by interfering with the internal structures of the nose, the nose skin is closed over the nose like a cover. Since the stitches will be located on the lower part of the nose, we can say that, in fact, rhinoplasty operations performed with open technique are almost scarless operations.
What You Need To Know About Rhinoplasty Surgery
• Rhinoplasty operations are procedures performed under general anesthesia. Although they are reliable and comfortable applications, they are not suitable operations for local anesthesia, especially considering the psychology of the patient.
• We prefer the patient to stay in the hospital for 1 day after the operation. Thus, the first 24 hours after the operation are spent with maximum comfort and under surveillance.
• Rhinoplasty operations are completed in an average of 2 hours, depending on the scope of the procedure and the intervention.
• After rhinoplasty, tampons are placed in the nose to prevent possible leaks. Although this may seem like a frightening experience to many, the tampons used in rhinoplasty have become much more comfortable thanks to the developing medical technologies. Today, silicone tampons are preferred instead of cotton as in the past. Since these tampons can be removed from the nose very easily due to their structure, situations such as fainting and pain are not encountered as feared. One week after the operation, the tampons are removed at the first control.
• In addition to the tampon, a plastic splint is placed on the upper surface of the nose after the operation in rhinoplasty surgeries. This splint helps the nose maintain its new form. Again, this splint is taken 1 week after the operation. If the open technique is preferred in the patient, sutures are also removed in the same control examination.
• Although the operation is comfortable, patients may complain of mild pain after the operation. However, these pains can be easily controlled with painkillers.
• In the first 2-3 days following the rhinoplasty surgery, bruising and swelling may be seen around the nose and under the eyes. This is an expected situation. This process is taken under control with ice compresses and creams applied.
• In the first week following your nose surgery, you may experience partial difficulties in breathing due to nasal crusting. With the special nasal sprays we recommend to our patients, we help the nasal crusts to soften and the nose to be cleaned easily and to perform the breathing function easily.
• About 2 weeks after the operation, since the sensitivity due to the operation in the nose is partially reduced, we tell our patients how to massage their noses during this period, and we help the edema to pass in a shorter time by massaging them at intervals.
• Swelling is expected in the first month following the operation. It takes about 6 months to 1 year for the nose to take its final shape.
• It is of great importance that patients comply with the control periods after the operation. In addition to the first week after the operation, we also check our patients in the 1st month, 6th month and 1st year.
Return To Social Life After Rhinoplasty
Although it is a comfortable operation, there are some points that you should pay attention to when returning to the routines of your social life after rhinoplasty. For example, you should pay attention to the cleanliness of the tampons placed immediately after the operation in the first week. However, you should avoid doing sports for the first 1 month, and you should not wear glasses as it will put pressure on the nose.
Again, during this period, activities such as pool, sauna, sea, or solarium that may cause edema and create a feeling of discomfort should be avoided.
You can return to social life while wearing a splint approximately 4-5 days after the surgery, but you should not carry heavy items and situations where the head will be in a tilted position should be avoided. These activities can cause bleeding in your nose.
Even if it is for cleaning the nose, steam bath should be avoided for the first month after the operation.
At the end of the first week, after removing the splint, you can take a shower with warm water without touching your face area. Since the use of hot water will cause a possible steam in the bathroom, warm water should be preferred. From the 2nd week, the head and body can be washed completely, provided that attention is paid to the face area.
What Is A Nose Massage And How Is It Done?
Nasal massage is an activity we recommend to our patients after rhinoplasty. Swelling and edema formation in the nose after rhinoplasty is an expected situation. With the nasal massage performed with light touches and using creams, these edemas and swellings are gone in a shorter time.
Nose massage should be done using a cream. Thanks to the lubricity provided by the cream on the skin surface, the nose and its surroundings can be massaged without difficulty. Although the massage is generally done from the bottom up, the same massage is not performed after every nose surgery. Nasal massage varies according to the technique used in the operation.
